Privacy Policy

The General Data Protection Regulation (GDPR) is concerned with the Personal Information about you that I collectstore, and share

 

Personal Information I will collect for the purposes of our work together:

  • Name, address, telephone and mobile number
  • Email address
  • Gender (or preferred identity)
  • Age / Date of Birth
  • GP and any other healthcare professionals involvement in your care
  • Marital status/ family relationships
  • Occupation
  • Medical conditions
  • Prescribed medication
  • Clinical session notes

 

How I will store your Personal Information

Paper records will be stored in in locked filing cabinet. These will involve all or some of the following:

  • Contact Sheet
  • Contract / Consent Form / GDPR Agreement
  • Assessments and assessment record
  • Session notes
  • Hard copies of any letters or correspondence regarding your care
  • Completed questionnaires

Electronic records

  • Personal information required for administration (invoices and financial records) and correspondence with other health care professionals or yourself, such as letters, will be stored on a password protected encrypted memory stick.
  • Your telephone number may be stored anonymously on my mobile phone.  This will allow me to contact you or exchange messages via SMS, but the number will not be associated with your name.
  • Your email address and correspondence will be stored in my secure email account by nature of you contacting me.   I use an encrypted email system to send and receive emails that contain your personal information or correspondence with other healthcare professions or health insurance companies.
  • No personal information is stored on my website, other than to momentarily collect and send it to my email account for the purposes of our initial contact.
  • No information will be stored on a portable laptop or desktop computer.

 

 

How I may process or share your Personal Information

Your information is completely confidential. I will not share your data with any third party without your permission unless the following circumstances occur:

  • If there is deemed to be a risk to yourself or others then I have a duty of care to inform and liaise with relevant third parties, such as your GP or social services. 

 

Your Rights

You have the right:

  • to be informed of the information I hold
  • to see the information that I hold about you (free of charge for the initial request)
  • to rectify any inaccurate or incomplete personal information
  • to withdraw consent to me using and holding your personal information
  • to request that your personal information be erased (though I can decline whilst the information is needed for me to practice lawfully & competently).

 

Erasing your Information

I will hold your written information and financial records for up to seven years after the date of your last session with me. This is so that I have a reference of our work should you return to therapy in the future, and to comply with financial law.  After this time has passed, I will shred the written information and delete all records.
